Population Health Final Project

The purpose of this project is for the learner to apply nursing skills and processes with a population focus rather than an individual focus. A broader population perspective allows us to take a more holistic and upstream approach in order to find solutions to health problems and promote health in the individuals and communities we serve.

This assignment allows you the opportunity to perform course objectives in “real life” by assessing a community as client, arrive at a community diagnosis, propose interventions, and decide how you would measure the outcomes. You will relate your project to overarching public health goals and Healthy People 2030 Objectives and Goals.

 The Final project will be a group presentation on VoiceThread during finals week. Your presentation will have the following components:

Introduction- 10%

Describe the community. This section is similar to the demographic information about a patient. For the community, it is a demographic description including Place, People and Function. Include at least 3 descriptors of Place, 3 for People/person, and 3 for Function.

Assessment- 20%

Community assessment is similar to what you do with an individual in collecting data and information to better understand the person, health, strengths, and goals. For a community, it is a process of identifying needs, clarifying problems, and identifying strengths and resources.

Assessing community requires:

  1. Data collection- each of the following needs to be included in your assessment.
    1. Existing, readily available data such as age of residents, gender distribution, socioeconomic characteristics, racial distribution, vital statistics, community institutions, health care services
    2. Collection of already reported data, such as minutes to applicable meetings, community programs, ongoing health surveys such as BRFSS or YBRSS(Behavioral [or Youth] Risk Factor Surveillance System Report), CDC website statistics, etc. related to topic. Use at least two sources for your community data.
    3. Windshield Survey- Use the elements of windshield survey and the descriptions as a guide as you drive or walk through the defined community. Summarize the description in narrative or table format, describing each of the elements. Some of the information needs to be obtained by gathering secondary data (statistics), which can be in column or table format.
    4. 2 key informant interviews- directed talks with selected community members, which include, informal leaders such as a group member who has status in that community, church deacon, school nurse, social workers or other community member that represent or work with your community of interest. Summarize each informant interview. Questions should be similar for each informant.
    5. Literature review- What has been written about similar populations as yours? What is the research, identified problems and potential interventions about this type of population? Include at least 2 current evidence-based, peer reviewed Nursing and/or Public Health research journal sources, no more than 5 years old, applicable to your population.

  1. Analysis of data, Identifying the Community Problem, narrative format
    1. From the composite database information collected above discuss in narrative format:

What are the strengths and problems for this community?

What are the risk factors?

What factors contribute to the problems?

What happens if the problem is not solved?

    1. Decide which community problem to focus on, and state why you chose this one. (Use the Healthy People 2030 Objectives related to the problem you decide to focus on).

Population problem statement 10%

Make a problem statement for the community that you have assessed. This should be the population health issue that you will address in the planning section of your project. Include a rationale for the problem you have chosen and which Healthy People 2030 objective this problem relates to.

Planning: 20%

  1. State Goals/objectives Use Healthy People 2030 as your resource here, but customize your goal to your specific population. State two SMART goals, a short-term goal and a long-term goal for your stated problem.

  1. Identify Intervention Activities for each goal. Identify who will do what. Identify the Nurse’s Role in the interventions. Include Evidence Based Guidelines (EBP Guidelines). Identify Primary, Secondary and Tertiary interventions related to this problem.

Evaluation of the Interventions and Outcomes 10%

Interventions do not need to be implemented, but you do need to determine how you would measure their effectiveness. Gather ideas from Healthy People 2030 that has measurements for their objectives. Describe evaluation methods Including sources of information for evaluating whether the goals were met. Discuss ongoing assessment of the population to determine if desired outcomes are sustained.

Online Presentation 20%

This presentation is asynchronous and will be shared using VoiceThread by adding slides and video. The presentation should be engaging and visually appealing, and clearly communicate the elements of each section of the project. Presenting should be equally shared among team members. The video for each section of the presentation may vary in length but should be limited to 5 minutes. For example, I would expect the introduction and problem statement videos to be shorter than the assessment and Interventions sections.

Peer Feedback on VoiceThread 10%

Each individual is required view each presentation and then choose three presentations to make a video comment. In your comment you may ask a pertinent question, share your own experience that relates to the presentation or describe an element of the presentation that was most impactful for your learning.
